Responsibility for processing your personal data

‍OptimumChiropractie】Optimum Chiropractie uses other organizations to perform processing on your data. You should think of software providers to enroll you in treatments, process your bill and schedule appointments. Optimum Chiropractie remains responsible for your data at all times and will ensure that these organizations process your data according to the requirements of the AVG.

You consent to the storage and processing of your data by having your care performed by Optimum Chiropractie.

Optimum Chiropractie processes your personal data because you use our services and/or you provide them to us yourself. Below is an overview of the personal data we process:

  • Gender

  • Date of birth

  • Address details

  • Phone number

  • Email address

Special and/or sensitive personal data we process Optimum Chiropractie processes the following special and/or sensitive personal data about you:

  • Health data

  • Citizen service number (BSN).

For what purpose and on what basis do we process personal data


Optimum Chiropractie processes your personal data for the following purposes:

  • Sending our newsletter and/or promotional flyer

  • Calling or emailing you if necessary to perform our services

  • To be able to perform our services to the best of our ability as therapists.

  • Optimum Chiropractie also processes personal data as we are legally required to do so by health insurance companies.

How long we keep personal data

‍OptimumChiropractie】Optimum Chiropractie will not retain your personal data longer than is strictly necessary to fulfill the purposes for which your data are collected. We use the following retention periods according to the Medical Treatment Agreement Act (WGBO) being twenty years `or so much longer as follows from the care of a good care provider`.

Sharing personal data with third parties

‍OptimumChiropractie】Optimum Chiropractie only provides information to third parties as necessary to fulfill our agreement with you or to comply with a legal obligation. Your information will not be shared with third parties without your written consent, unless there is a legal obligation to do so.

View, modify or delete data

​

‍Youhave the right to access, correct or delete your personal data. In addition, you have the right to withdraw your possible consent to data processing or object to the processing of your personal data by Optimum Chiropractie and you have the right to data portability. This means that you can make a request to us to send the personal data that we hold about you in a computer file to you or another organization named by you. You can send a request for inspection, correction, deletion, data transfer of your personal data or a request to withdraw your consent or objection to the processing of your personal data to francis@optimum-chiropractie.nl. To make sure that the request for inspection is made by you, we ask you to send a copy of your proof of identity with the request. In this copy, black out your passport photo, MRZ (machine readable zone, the strip of numbers at the bottom of the passport), passport number and Citizen Service Number (BSN). This is to protect your privacy. We will respond to your request as soon as possible, but within four weeks.

You may withdraw your consent to the processing of your personal data. Optimum Chiropractie may then no longer be able to provide responsible care and treatments will be discontinued. Your data will then be stored in an archive, invisible to normal use.

‍OptimumChiropractie】Optimum Chiropractie takes the protection of your data seriously and takes appropriate measures to counter abuse, loss, unauthorized access, unwanted disclosure and unauthorized modification.
